Oblivion sounds pretty damn good, actually, in Jaeger's current state of mind. Maybe not permanent oblivion, but just not having to think or feel.
Yelena has seen him in dreams, who he was before the scars, before the ruin. Would she be that much more repelled by the horror he's become? He thinks so. The misery is there, a cloud of bleak despair that won't lift, that has no end, and the fact that she's witnessing it only makes it worse.
He can't let go. Even with her hands clutched around his wrists (so awfully close to The Scars, can she feel them? Those slick, shiny trails that speak of a sadist's aesthetic).
"I can't." It's a simple statement, but heavy with desperation. It's so clear in his mind: he simply cannot conceive of being able to let go of the lamb.
